EdReady Sign-up Form

EdReadyMT requires the following information to enroll a student in the free EdReady program. Double check the information you enter. If it is entered incorrectly, you may not receive your login information.

After you complete the form, please allow one week for your information to be processed. You will then receive an email with login information and instructions.

This information will help us enroll you in the EdReady module most appropriate for your current goals. In case any additional information is needed, we may contact you with some follow-up questions before you are enrolled in EdReady. Important: If you do not receive your login information within one week of using this form, check your junk mail folder. If you cannot find your login information in your inbox or junk mail, contact us immediately.

Gender
If you answered "to prepare for a specific math course" or "to prepare for a course that requires math" for the previous question, what course are you preparing for? If you answered "as a skills placement test," what course are you trying to place into (M 105, M 115, or M 121 Plus)? If you answered anything else, please skip to the next question.
If you are unsure about your major, please provide your potential areas of interest.
If you do not have any placement information or you are unsure of your placement, please write "I don't know."
Please provide the last math class you took, approximately when it was taken, and whether you took it in high school or college.
